A Case of Cogentin Intoxication




Abstract
This study was undertaken to present an example of the manifestations of the central nervous system and peripheral toxicity in a patient, who used 120 mg of Cogentin for suicidal purpose. The clinical problem is summarized below.
1) One can consider two possibilities regarding the development of the seizure and comatose states. The first of the possibilities may be due to hyponatremia; whereas, the second may be due to the central toxic effect from benztropine intoxication.
2) Since physostigmine is not produced in this country, there was difficulty in obtaining the drug improving the comatose state of this patient.
3) In general, the symptoms of benztropine toxicity are spontaneously diminished within a period of 24-48 hours after drug ingestion. In the given example, the vital sign and physical signs improved immediately, but the recovery of consciousness was established after 96 hours.
4) Aphasic state was gradually improved, according to the follow-up study (by telephone) through the present, the patient began using nouns and pronouns about late July(25 days after ingestion). Although the patient started using adjectives and constructed relatively proper compositions, beginning in late September, the patient still spoke with the pronunciation and intonation like those of a foreigner.
5) According to the risk-rescue rating regarding this case, the risk score of 4 points was high moderate and the rescue score of 3 points was moderate. In other words, although the suicidal attempt was relatively severe, the opportunity for resuscitation signified one of moderateness.
However, in this case, the circumstances under which life was revived may be considered by two different approaches. The first point being that benztropine in relatively large doses (less than 120 mg) may not be fatal. The other point being that life was possibly prolonged and revived by prompt emergency supportive treatment against the toxic symptoms due to the metabolic disturbance caused by the action of benztropine.
